Attached is the Fee Assessment/Refund Schedule for the 2022-23 school year.
Beginning July 18, 2022, all usernames for the Perform system will be district emails.
This change will allow the evaluation department to better assist with evaluations and location of educators in the district and also take care of some glitches that have been encountered. Please feel free to reach out with any questions or concerns to Rebecca Lee, (801) 567-8369.
Note: Passwords that have been created will not change.
The mileage reimbursement rate is based upon the IRS rate. Effective July 1, 2022, the reimbursement rate is $0.625 per mile. Please use the attached form when submitting for mileage reimbursement. All 2022 mileage reimbursement requests should be submitted as soon as possible. Click HERE to access the mileage form online.

Networking Meetings for Secondary Media Specialists
In order to prepare and train secondary library media specialists to effectively provide services to their schools and to build consistency across the District’s library programs, several networking meetings have been scheduled for the 2022-23 school year. The participation of your media specialists in these meetings is appreciated.
The meetings will be held on the following dates from 8:00 a.m.-11:30 a.m.:

Some reminders for renting a vehicle when traveling for District business:
1) Indicate the rental needs such as size of vehicle and length of time on the initial travel request form. This is how the supervisor approves the use and expense of a rental vehicle before the travel occurs.
2) Many hotels will charge to park a vehicle. Remember to include those charges in your cost estimates on the travel request form.
3) There are two state contracts for rental vehicles through Hertz and Enterprise. By renting through a state contract, there are liability protections for the driver. The District Travel Buyer will, upon request, make the car rental reservations under a state contract.
4) Rental vehicles reserved outside a state contract may add additional cost and/or personal liability to the driver. Therefore, it is strongly recommended rental vehicles be reserved on a state contract through the District Travel Buyer.
5) No rental vehicle expenses (reservations, parking, and fuel) can be prepaid by the Travel Buyer. These expenses must be paid by the person actually renting/driving the vehicle either using their P-Card or a personal credit card (for subsequent reimbursement).
6) Remember to keep your receipts when paying for pre-approved vehicle rentals.

Fee Waiver Materials 2022-23
The annual Certificate of Compliance report will be due October 31, 2022. October 1st of each year the Local Board Chair, District Superintendent, and each Principal will receive an email containing a personalized link to complete the Certificate of Compliance. The email will be titled Utah State Board of Education School Year 2022-2023 Certificate of Compliance. Each individual will have 30 days to complete the certificate. All USBE School Fees Notices and School Fee Waiver Applications can be found by clicking HERE.
Forms can be found under Grades K-6 and Grades 7-12 in multiple languages. The Fee Waiver Application, for both elementary and secondary was revised 4/4/2022. Use the current form when having parents fill out the application.
Attention: Students who previously showed verification of eligibility do not need to resubmit documentation. It may be necessary for the principal (or designee) to contact the previous school of transferring students for verification of fee waiver status. Kindergarten students coming into the school, new students to the District, and students with extenuating circumstances will still be required to submit documentation.

Workers' Compensation insurance costs are based on numbers submitted by schools. It is important that this information be accurate and complete.
PTA figures must be separate from other volunteer hours.
